The movie opens with Death Dealers (vampire warriors charged to fight and kill the Lycan enemy) Selene and Rigel trailing a pair of Lycans, (Raze and Trix), unaware that they are in turn stalking a human - Michael Corvin. Trailing them into a subway station, the two Death Dealers are spotted by Raze and a shoot-out ensues, with Rigel being killed by the Lycans. A woman is shot in the crossfire, and Michael tries to assist her. The two sides of monsters continue their fight, and the Lycans eventually retreat.
Arriving at Ordoghaz, Selene confronts Kahn, the chief Death Dealer, with her findings during the battle; but the regent Kraven blows off the significance of her claims. Selene decides to further investigate the ordeal alone. Meanwhile, in the Lycan lair, a scientist named Singe unsuccessfully seeks a match for the Corvinus Strain. They decide to try again in pursuit of Michael.
Selene finds Michael's apartment and demands answers. They are attacked by Lycan hordes and Michael is bitten by Lucian, who gathers a blood sample. Selene, unaware of him being bitten, aids Michael in escaping the building. Selene takes Michael to the vampire lair for safety, but he soon escapes when the other vampires figure out he's become a Lycan. Selene grows suspicious of Kraven and collects evidence to take to the library. She learns that Lucian is still alive and that he and Kraven are secret allies with each other. Selene decides to awaken Viktor.
Michael tries to seek aid from his friend at the hospital, but his tale of being bitten by a man leads to treachery on the part of his friend. He escapes again, attempting to meet with Selene and begging for answers. Selene drives out to meet Michael, where she sees that he had indeed been bitten. She explains the long history between Vampire and Lycan. Kraven and Erika run into Viktor, who demands to know what has happened, and learns that he has been awakened a century early.
Selene takes Michael to a safehouse. Michael mentions his nightmares and Selene explains they are actually Lucian's memories. Selene explains that her hatred of Lycans stems from her belief that they killed her family. Michael is captured by the other Lycans in an attempt to escape as a Vampire-Lycan firefight breaks out just outside the room from which he jumps. Selene ends up the only vampire survivor, and Singe the only Lycan survivor, still left on the scene after Pierce and Taylor haul Michael away.
Kraven tries to explain what has happened to Viktor right as Selene arrives with Singe. The captive Lycan reveals that the Lycans had been experimenting to combine the bloodlines of the two species via the Corvinus Strain, something they can only do with a descendant of Alexander Corvinus himself, which is why they targeted Michael. They wished to make a Hybrid so they could end the war. He also reveals Kraven's treachery, forcing Kraven to flee. Kahn arrives and informs Viktor of Amelia's fate in the shootout in the safehouse. In a rage, Viktor kills Singe. Viktor then turns to Selene and tells her all she has to do is kill Michael and she will be exonerated.
Michael wakes up strapped to a slab in Singe's lab. He is greeted by Lucian, who then extracts blood from Michael and injects it into himself. While Michael watches him, he recognizes the pendant Lucian wears, and suddenly, his nightmares make sense: he recalls the events of how Lucian's wife, Viktor's daughter Sonja, was murdered by Viktor out of fear that the infant in her womb would be a hybrid that would overthrow him. Viktor, it is revealed, started the war. Lucian explains the rest of the story to Michael.
Selene finds Michael and unties him, telling him to go despite his protests they'll kill her too. Lucian, crawling after being betrayed by Kraven, sees them together. Kraven shoots Michael full of silver nitrate bullets, beginning to kill him. Out of spite, Kraven tells Selene the truth that Viktor was the one who truly killed her family. Viktor couldn't kill Selene because she reminded him too much of Sonja, his daughter. Lucian distracts Kraven long enough to advise Selene to bite Michael, reminding her of Singe's words. She takes his advice and bites Michael. Kraven and Lucian then fight and Kraven finishes Lucian off, barely surviving himself. Selene confronts Viktor, who despite his guilt, defends all his actions.
Michael changes into his hybrid form battles with Viktor. Selene manages to get a lucky shot, killing Viktor by slicing his head in half. Selene picks up Sonja's pendant and gives it to Michael to keep. They walk out together, speechless and numb. Selene then narrates to the audience that she fears she will become a target following Viktor's death. Back in the crypt, Singe's blood pours across the floor until it reaches Marcus' sarcophagus. His eyes flutter open. First vampire blue then hybrid black....

The story opens in a flashback sequence to the year 1202 AD. An army of medieval warriors arrives at a ravaged village, led by the three vampire elders: Marcus Corvinus, Viktor, and Amelia. Viktor orders his troops to quickly burn the corpses of the slain villagers, but before the task is completed, the bodies awake and transform into werewolves. A bloody battle erupts, and Viktor orders Marcus to retreat. Viktor and Amelia take the rest of their troops into the woods, where they capture Marcus's brother William Corvinus, the first and most powerful werewolf. Marcus returns and angrily demands that William be turned over to his care. Viktor refuses, and orders instead that William will be imprisoned in a secret location forever.
The story then continues the present-day timeline, starting immediately after the events of the previous movie. Selene and Michael take shelter in a vampire safe house. Selene warns Michael that he can only feed on cloned blood, and that she needs to return to the mansion alone to keep Kraven from killing Marcus. What she doesn't realize is that Singe's blood had transformed Marcus into a hybrid. Marcus breaks out of the sarcophagus on his own and wipes out the entire remnant of Kraven's men, killing Kraven last as vengeance for Kraven's treachery. He gains clues as to the whereabouts of William's prison by drinking Kraven's blood.
Selene begins to head out on her way back to the mansion, but witnesses what she deduces to be Marcus flying overhead, possibly in pursuit of her. Michael ignores her warnings and tries to order some fish at a restaurant. He has a strong negative reaction to the food. The police and the media, due to Pierce and Taylor, believe that Michael is a wanted fugitive, and the police, upon seeing his face on the TV screen, immediately recognize him. He begs for them to stay away, but they attack anyway. He rushes away to avoid the temptation to prey on them. They fire rounds at him, but Selene intervenes and quickly disposes of the police.
Marcus, in Hybrid form, finds them. Selene desperately attempts to defend her actions, and Marcus blows off her comments about Viktor, agreeing that Viktor got what he deserved. He does argue that he believes Selene might be hiding something, and attacks her. Michael intervenes and protects her. They hijack a truck and escape from Marcus, but he vows to get his fangs on Selene's blood memories one way or another.
The sun starts to rise, burning Selene's hand and part of her face. Michael covers her up and steers the truck to an abandoned garage. After covering the windows, he leads her to a cargo container. She reveals that once out of the sunlight, her wounds healed quickly on their own. They settle down in their safe getaway, and a love scene ensues.
Lorenz Macaro, an elderly and imposing man, sends in a team of "Cleaners" to investigate the finds of all the dead bodies from the first movie. When Macaro examines Viktor's body, and finds a metal disk attached to his ribcage, just below the skin of his chest. Macaro fears the worst as he hears that Marcus' body was not amongst those found at the mansion.
Michael and Selene then set out to solve the mystery of the pendant that Marcus wanted from them so badly. Selene can only recall that she'd seen it before as a child, but knew not its significance. She decides they must try to travel to Andreas Tanis' hideout for answers, as he is the exiled historian who could help them out. They arrive to find that Tanis had been secretly involved in weapons trade with Lycans. After a battle with the Lycans and with Tanis' concubines, they force him to tell them as much as he can.
He reveals that Marcus, not Viktor, was the first vampire. One of the three sons of Alexander Corvinus, he was bitten by a bat, and became a vampire; his twin brother, William Corvinus, was bitten by a wolf and became a Lycan. Unlike later Lycans, William, and the Lycans created by him, are entirely animal, unable to take human form again. When William ran amok, Marcus approached Viktor, a warlord, as he was dying of old age, and offered him a deal: to turn him and his army into immortal vampires, in exchange for his help in tracking down and stopping William. Marcus intended to capture his brother and tame him; Viktor has different plans and orders William locked away forever. As time went on, Viktor came to dominate the vampire coven, and was falsely regarded as the first vampire. But Viktor was afraid to kill either Marcus or William, as Marcus had told him that doing so would lead to the immediate extinction of all the vampires or Lycans, whom Viktor had enslaved.
Selene's father is revealed to have been the architect who built William's prison; the amulet is one half of its key (the other half is the disk that Macaro took from Viktor's chest). When Lucian escaped captivity, taking the amulet with him, Viktor killed Selene's father and family. Selene's latent memories from her childhood are the map to finding the prison. Tanis then arranges a meeting between Selene, Michael, and "someone who could help stop Marcus." After they leave, Marcus arrives at the Monastery and questions Tanis, biting him, absorbing his memories, and killing him.
Arriving at the docks, Selene and Michael are taken to Macaro, whom Selene realizes is actually Alexander Corvinus. Since his sons turned, he has devoted his life to containing the vampire-lycan war, keeping it from escalating over to the mortal world. But despite Selene's entreaties, he refuses to help kill his own son, no matter what he's become. Their debate is broken when Marcus attacks the boat. Marcus and Michael fight, and Marcus rams his body down on a wooden stake, impaling and seemingly killing him. Taking the amulet, Marcus then grabs Selene and pins her, taking her blood and finding what he's after. In the ship's cabin, Marcus confronts his father. Alexander fails to convince Marcus to back down from his quest to free William. Marcus rants and muses becoming a god, and mortally wounds Alexander to get the second part to William's key. The Cleaners take Selene and Michael's body to Alexander, who begs Selene to take some of his blood for her own sake, promising it will make her "the future."
They arrive by helicopter too late, and find that Marcus has already freed William. A violent battle ensues, and the Cleaners are transformed into first-generation Lycans. Marcus manages to shift the stone door that he becomes trapped behind, and joins the fight. Michael suddenly regenerates and arises from his slumber to join the fight. As Selene and Michael try to kill William with aid from the helicopter's Minigun, Marcus grabs a rope dangling from the helicopter and yanks it down, causing it to crash inside the dungeon, blades still spinning.
Selene battles Marcus while Michael fights with William. Marcus is distracted when Michael gains the upper hand, and tears off the top of William's head In rage, he impales Selene through the heart with one of his talons. But she survives, pulls out the talon, breaks it off, and rams it up into Marcus's head. As a coup de grace, she sends him flying into the helicopter blades, cutting him into raining chunks.
Selene and Michael find themselves the only monstrous beings left alive in the caverns, and are amused that sunlight no longer affects her. Selene states that she fears dark days are ahead, as the lines between the species have become blurred. She states that she is at least optimistic that she won't have to face the future alone.
